Rural Health Transformation (RHT) Program
The Rural Health Transformation (RHT) Program is an unprecedented federal initiative providing $50 billion in funding over five years (2026–2030).
Established under the One Big Beautiful Bill Act (Public Law 119-21), the program is designed to modernize rural healthcare infrastructure, with $10 billion available nationally each year.
In the first year of the program (2026), state awards average approximately $200 million, ranging from roughly $147 million (New Jersey) to over $281 million (Texas). September 2027 is the mandatory deadline for completing FY 2026-approved projects.
The Rural Health Transformation (RHT) Program is overseen by the Centers for Medicare & Medicaid Services (CMS), which is an agency within the U.S. Department of Health and Human Services (HHS).
While states receive the awards directly, the program explicitly targets "Modernizing Rural Health Infrastructure and Technology" as a core pillar. Facilities are encouraged to align their technology roadmaps with state-specific transformation plans to access these funds through state-administered grants and cooperative agreements.
